Media Permission Form: This letter is to request permission (in accordance with Policy 815.1) for your child’s image, voice, video work and/or first name to be published publicly.
Parent Permission Slip: All students going on school trips will receive a permission slip from their teacher to go on a trip. This form must be completed in its entirety, signed by a parent or guardian and returned to school before a student is allowed to attend any trip.
